## Job description

Role Overview: The Behavioral Health Assessor-Licensed conducts comprehensive face-to-face and tele-mental health assessments for patients experiencing psychological, behavioral, or substance-related disturbances. Working under the supervision of a manager, they collaborate with physicians and care teams to develop individualized treatment plans, assess patient needs, and determine appropriate levels of care. The role involves coordinating discharge plans, referring patients to appropriate services, and documenting assessments and interventions accurately in electronic records. Key Responsibilities Perform clinical assessments, including suicide risk evaluations, in emergency and acute care settings. Collaborate with multidisciplinary teams and support systems to create safe, effective care plans. Coordinate patient discharge and referrals, ensuring barriers to care are addressed. Develop safety plans, educate patients and families, and incorporate cultural considerations into care. Maintain comprehensive documentation of assessments, safety plans, and contact with support systems. Qualifications & Skills Master's Degree in Social Work or Counseling from an accredited program. Licensure as a Licensed Associate Professional Counselor or Licensed Master Social Worker in Georgia. Minimum of 3 years of experience working with adult and child populations, preferably with behavioral health patients. Effective verbal and written communication skills, ability to think abstractly, and knowledge of behavioral health best practices. Proficiency in Word, Excel, and telemedicine technology. Additional Requirements: Certifications in BLS, workplace violence prevention, and licensure upon hire are required.
