They collaborate with multidisciplinary teams to develop, implement, and revise individualized care plans, ensuring patient goals are met. Key Responsibilities Deliver direct patient care through assessments, therapeutic exercises, and treatments, adjusting frequency based on patient progress and physician orders. Coordinate care by developing and updating the plan of care, communicating with team members, and reporting patient status changes to physicians. Educate patients and caregivers to promote independence and support recovery, utilizing verbal and written tools. Participate in staff education, onboarding, and continuing education to maintain clinical competency. Ensure accurate documentation of interventions, adhere to regulatory guidelines, and communicate effectively within the care team. Qualifications & Skills Graduate of an accredited Occupational Therapy program. Active licensure as an Occupational Therapist and Basic Life Support certification. Minimum of one year of full-time occupational therapy experience; home health experience preferred. Strong assessment, rehabilitative, communication, organizational, and time management skills.
