Educate patients and caregivers to promote independence and support recovery. Coordinate services with team members, report changes to physicians, and support multidisciplinary collaboration. Participate in staff training, continuing education, and professional development activities. Accurately document interventions, communicate patient status, and adhere to regulatory guidelines and documentation standards. Qualifications & Skills Graduation from an accredited Occupational Therapy program. Valid Occupational Therapist license and Basic Life Support (BLS) certifications. Minimum of 1 year of full-time occupational therapy experience; home health experience preferred. Strong assessment, rehabilitative, and critical care skills. Excellent communication, organizational, and time-management skills. Proficiency with computer software, including home health patient management tools, email, and internet.
