They collaborate with multidisciplinary teams to develop and revise individualized care plans, ensuring all goals are met. Key Responsibilities Manage patient care from admission to discharge, performing therapeutic assessments, exercises, and treatments in the patient's home. Develop, coordinate, and adjust care plans in accordance with physician orders and patient needs. Educate patients and caregivers to promote independence and support recovery, utilizing verbal and written communication tools. Participate in staff training, continuing education, and maintain current clinical knowledge. Accurately document interventions, communicate patient status to physicians, and adhere to regulatory guidelines. Qualifications & Skills Graduate of an accredited Occupational Therapy program. Licenses: Occupational Therapist, BLS certification, Georgia Driver’s License, and private vehicle insurance. Minimum of 1 year full-time OT experience; home health experience preferred. Strong assessment, problem-solving, organizational, and communication skills.
