## Job description

Role Overview: The Occupational Therapist provides age-appropriate rehabilitative interventions to patients with physical, emotional, developmental, psychosocial, and educational needs in a home health setting. They collaborate with multidisciplinary teams to develop and adjust care plans, ensuring all patient goals are met. Key Responsibilities Assess and manage patients' rehabilitative needs, delivering therapeutic exercises, education, and treatments. Develop, revise, and coordinate individualized care plans within medical guidelines. Provide patient and caregiver education to promote independence and support recovery. Communicate patient progress and changes with physicians and team members. Complete accurate documentation in compliance with regulatory standards and maintain timely records. Participate in staff training, ongoing education, and special projects to stay current in clinical practices. Qualifications & Skills Graduate of an accredited Occupational Therapy program. Licenses and certifications including BLS, OT licensure, Georgia driver’s license, and private vehicle insurance. Minimum of 1 year full-time occupational therapy experience; home health experience preferred. Strong assessment, rehabilitative, and critical care skills. Excellent communication, organizational, problem-solving, and time management skills. Proficiency in computer use, home health software, and understanding of healthcare regulations.
