The OT collaborates with physicians and families to establish and evaluate individualized care plans, including discharge planning and community resource referrals. Key Responsibilities Implement and adjust treatment plans; select appropriate activities; educate patients and families. Conduct assessments, establish goals, and develop realistic discharge plans. Document evaluations, treatment sessions, and discharge instructions per departmental standards. Maintain compliance with regulatory standards, ethical practices, and confidentiality. Provide coverage for a minimum of two weekend days per month and one holiday annually. Participate in staff meetings, ongoing education, and training to maintain licensure and professional growth. Qualifications & Skills Bachelor’s or Master’s degree in Occupational Therapy. Licensed as an Occupational Therapist and certified in BLS/ARC-BLS. Effective communication, customer service orientation, teamwork, and basic computer skills.
