## Job description

Role: A licensed physical therapist responsible for assessing, planning, and implementing rehabilitative programs to improve mobility, reduce pain, and prevent deformity in patients with disease or injury. Services are delivered across the care continuum with a focus on quality, cost-effectiveness, and regulatory compliance. Key Responsibilities Develop and revise treatment plans in collaboration with patients and their families. Provide patient education, discharge instructions, follow-up, and community resource referrals. Conduct patient assessments, establish treatment goals, and evaluate progress. Document evaluations, treatment sessions, and discharge plans per departmental protocols. Act as a resource for staff, families, and physicians regarding rehabilitation services and equipment. Maintain professionalism, confidentiality, and adhere to ethical standards. Communicate effectively, participate in staff meetings, and organize workload efficiently. Foster a positive patient and team environment, applying customer service principles like AIDET. Engage in ongoing professional development, complete mandatory trainings, and maintain licensure. Qualifications Bachelor’s, Master’s, or Doctorate in Physical Therapy. Current Georgia state licensure and certifications including PT license and BLS. Effective communication, customer service focus, team orientation, and basic computer skills.
