## Job description

Role Overview: Responsible for evaluating and treating patients in Rehab and LTAC hospitals, documenting in EMR, and educating patients, families, and staff. Collaborates with interdisciplinary teams, communicates patient progress, manages equipment orders, and liaises with vendors and outside professionals. Key Responsibilities Conducts initial and discharge assessments, sets goals, and documents treatment plans. Develops and delivers timely, goal-oriented therapy interventions. Identifies educational needs and provides PRN services. Participates as a team member and mentors interns as needed. Engages in skills checks and ongoing training. Qualifications & Skills Bachelor's Degree in Physical Therapy required; Doctoral preferred. GA Physical Therapy license or eligibility. BLS certification from the American Heart Association. Minimum of 1-year experience in rehab settings such as SNF, LTACH, or acute care preferred. Basic computer literacy.
