They supervise and coordinate care with multidisciplinary teams, including RNs, LPNs, Therapists, and aides, ensuring goals are met to promote patient recovery and independence. Key Responsibilities Assess and manage patient needs following physician orders, adjusting care as needed. Develop and revise care plans, supervise PTAs and Home Health Aides, and ensure effective communication with physicians and team members. Educate patients and caregivers to promote self-care and recovery outcomes. Participate in staff training, continuing education, and performance improvement activities. Ensure compliance with regulatory guidelines, complete documentation accurately and timely, and meet productivity standards. Qualifications & Skills Graduate of an accredited physical therapy program; licensed as a Physical Therapist. Minimum of 1 year full-time experience, with preferred home health background. Required certifications include BLS and a valid Georgia driver’s license with insurance.
