The therapist communicates effectively with physicians and team members, documents accurately, and adheres to compliance standards. Qualifications & Skills Graduate of an accredited physical therapy program. Valid Physical Therapist license and BLS certification. Minimum of 1 year full-time PT experience, preferably in home health. Strong assessment, rehabilitation, communication, and organizational skills. Proficiency in home health operations, documentation, and regulatory requirements; OASIS knowledge preferred. Valid driver's license, private vehicle insurance, and ability to work independently in patients' homes. Key Duties Assess, plan, and manage patient care from admission to discharge. Supervise and coordinate multidisciplinary services to meet patient goals. Provide patient and caregiver education to foster self-care and recovery.
