## Job description

Role Summary: This position involves providing home-based physical therapy care to patients across multiple counties, focusing on recovery and rehabilitation. The therapist will assess, develop, and supervise individualized care plans, coordinate with multidisciplinary teams, and ensure positive patient outcomes. Key Responsibilities Deliver direct patient care, managing 25-35 patients in their homes and adjusting treatment as needed. Supervise and coordinate with PTAs and home health aides throughout the care episode. Communicate with physicians, patients, families, and team members to develop and update care plans. Provide patient and caregiver education to promote independence and self-care. Participate in staff education, continuing education, and quality improvement initiatives. Ensure compliance with regulatory guidelines and complete documentation accurately and timely. Contribute to performance improvement activities and attend team conferences. Qualifications & Skills Graduate of an accredited physical therapy program. Valid Physical Therapist license and BLS certification. Minimum of 1 year of full-time PT experience, preferably in home health. Strong assessment, rehabilitation, organization, and communication skills. Proficiency with computer systems, home health software, and understanding of regulatory and reimbursement processes. Additional Requirements: Valid driver’s license, private vehicle insurance, and willingness to participate in rotation duties, including weekends and call as needed.
