They coordinate all aspects of the patient's rehabilitative episode, supervise support staff, and communicate with healthcare team members to develop and implement effective care plans. Key Responsibilities Assess, develop, and manage patient care plans following physician orders, adjusting visit frequency based on clinical status. Supervise PTAs and Home Health Aides, ensuring quality care delivery across the continuum. Provide patient and caregiver education to promote self-care and independence. Communicate patient progress and changes with physicians, participate in team conferences, and ensure documentation compliance. Adhere to regulatory guidelines, participate in performance improvement activities, and maintain professional development. Qualifications & Skills Graduate of an accredited physical therapy program. Licenses and certifications including BLS Instructor, Physical Therapist license, and valid driver's license with private vehicle insurance. Minimum of 1 year of full-time physical therapy experience, preferably in home health. Strong assessment, rehabilitation, communication, organizational, and problem-solving skills. Proficiency with home health software, electronic documentation, and understanding of home health regulations.
