## Job description

Role Overview: The Physical Therapist provides primary rehabilitative care to approximately 25-35 patients in home settings, addressing their physical, emotional, developmental, psychosocial, and educational needs. They coordinate all rehabilitative disciplines throughout the care episode and supervise PTAs and Home Health Aides. Key Responsibilities Assess, develop, and manage individualized care plans in collaboration with the care team and physicians. Perform routine and complex therapy procedures, adjusting visit schedules based on patient needs. Coordinate services across disciplines, revise care plans, and ensure goals are met for positive outcomes. Educate patients and caregivers to promote self-care and independence. Communicate patient status changes to physicians and participate in team conferences. Ensure compliance with regulatory guidelines, complete documentation accurately, and participate in performance improvement activities. Contribute to staff education, meet productivity standards, and perform assigned duties, including weekend and call rotations. Qualifications & Skills Graduate of an accredited physical therapy program. Licenses and certifications: BLS/Instructor, Physical Therapist license, valid driver’s license, and private vehicle insurance. Minimum of 1 year of full-time physical therapy experience; home health experience preferred. Strong assessment, rehabilitation, leadership, organizational, and communication skills. Proficiency with Windows-based software, home health operations, and regulatory processes; OASIS knowledge preferred.
