Responsibilities include assessing patient needs, developing and managing individualized care plans, and coordinating with interdisciplinary teams to ensure positive outcomes. They supervise PTAs and Home Health Aides, ensuring quality care and adherence to physician orders. Key Duties Conduct comprehensive patient assessments and develop tailored treatment plans. Manage patient progress from admission to discharge, adjusting care as needed. Supervise and coordinate care with PTAs, aides, and other team members. Educate patients and caregivers to promote self-care and independence. Communicate effectively with physicians and team members regarding patient status. Ensure compliance with regulatory guidelines and complete documentation accurately and timely. Participate in performance improvement activities and staff training. Qualifications & Skills Graduate of an accredited physical therapy program. Valid Physical Therapist license and BLS certification. Minimum of 1 year of full-time PT experience, home health experience preferred. Strong assessment, rehabilitation, and communication skills. Proficiency with home health software, documentation, and regulatory standards. Valid driver's license, private vehicle insurance, and ability to travel within assigned counties. Additional Requirements Positive leadership and organizational skills. Ability to follow directions and operate within healthcare regulations.
