They supervise and coordinate care with multidisciplinary teams, including RNs, LPNs, Therapists, Social Workers, and Aides, to develop, implement, and revise individualized care plans that meet physician goals. Key Responsibilities: Assess and manage patients' needs, following physician orders, from admission to discharge. Supervise PTAs and Home Health Aides, ensuring quality care delivery. Educate patients and caregivers to promote independence and self-care. Communicate patient status updates to physicians and team members. Ensure compliance with regulatory guidelines and complete documentation accurately and timely. Participate in performance improvement activities and team conferences. Maintain current clinical knowledge through ongoing education and training. Qualifications & Skills: Graduate of an accredited physical therapy program. Licensed as a Physical Therapist; BLS certification required. Minimum of 1 year of full-time PT experience, preferably in home health. Strong assessment, rehabilitation, communication, and organizational skills. Proficiency with Windows-based systems, home health software, and understanding of home health regulations.
