Responsibilities include assessing patient needs, delivering age-appropriate interventions, supervising PTAs and aides, and ensuring positive outcomes through effective communication with physicians, patients, and families. Core Duties: Conduct patient assessments, develop and revise care plans, manage therapy procedures, and coordinate services throughout the episode of care. Patient Education: Educate patients and caregivers to promote self-care and independence, utilizing verbal and written tools. Regulatory Compliance: Ensure documentation accuracy, adhere to CMS, state, and Joint Commission guidelines, and participate in performance improvement activities. Team Collaboration: Communicate patient progress, participate in team conferences, and contribute to department and system-wide initiatives. Additional Tasks: Support staff education, meet productivity standards, and participate in weekend and call rotations as needed. Qualifications: Must have a graduate degree from an accredited physical therapy program, a valid Georgia license, BLS certification, and a minimum of one year of full-time PT experience, preferably in home health.
