Responsibilities include collaborating with physicians to develop and evaluate treatment plans, adjusting care based on patient and family feedback, and providing education and discharge instructions. The role involves selecting appropriate treatment activities, documenting evaluations and care progress, and acting as a resource for staff, families, and physicians regarding rehabilitation services and equipment. Key Duties Conduct patient assessments and develop care plans with input from patients, families, and physicians. Implement and adjust physical therapy treatments to meet patient goals. Provide ongoing education, discharge instructions, and community resource referrals. Document evaluations, treatment sessions, and discharge records per departmental guidelines. Maintain professionalism by adhering to ethical standards, confidentiality, and policies. Engage in team collaboration, meetings, training, and process improvements. Lead with excellent customer service, applying AIDET principles, and fostering a positive environment. Participate in professional development, complete required competencies, and train new staff. Qualifications & Skills Graduate from an accredited physical therapy program with a Bachelor's, Master's, or Doctorate degree. Current Physical Therapist license and BLS certification (including instructor and provisional certifications). Effective communication, customer service focus, teamwork, and basic computer skills.
