## Job description

Role Overview: This position involves the evaluation and treatment of patients primarily in Rehab and LTAC hospitals, including documentation, patient and staff education, and communication with team members and external professionals. Responsibilities include conducting initial and discharge evaluations, developing and implementing treatment plans to achieve functional goals, and serving as a mentor for interns. Key Duties Perform assessments, set goals, and document appropriately. Create and deliver effective treatment plans in a timely manner. Identify and address patient education needs. Participate as an active team member in interdisciplinary discussions. Serve as a clinical instructor/mentor for interns as needed. Contribute to skills assessments and departmental initiatives. Qualifications & Skills Bachelor's Degree in Physical Therapy required; Doctoral degree preferred. Minimum of 1 year experience in Rehab, SNF, LTACH, or Acute settings preferred. Licensure or eligibility for GA PT license, with BLS certification. Basic computer literacy required.
