## Job description

Role Overview: Provides evaluation and treatment for patients primarily in rehab and LTAC hospitals, ensuring comprehensive care aligned with patient needs. Key Responsibilities Conducts initial and discharge evaluations, develops treatment plans, and documents progress accurately. Implements timely and effective therapy to achieve functional goals. Identifies educational needs for patients, families, and staff, providing PRN as needed. Participates actively as part of an interdisciplinary team and communicates patient progress in clinical conferences. Serves as a mentor or clinical instructor for interns and participates in skills assessments. Coordinates with outside physicians, vendors, and other professionals, manages equipment orders, and maintains documentation in the EMR. Qualifications & Skills Bachelor's Degree in Physical Therapy required; Doctoral Degree preferred. Minimum of 1 year experience in rehab, SNF, LTACH, or acute settings preferred. GA PT license or eligibility, BLS certification required. Basic computer literacy necessary.
