Responsibilities include conducting initial and discharge evaluations, developing and implementing treatment plans to meet functional goals, and documenting progress in the EMR. Communicates effectively with patients, families, staff, physicians, and vendors, and participates in interdisciplinary team activities. Key Duties Perform initial and discharge assessments, set treatment goals, and document appropriately. Create and deliver effective therapy plans to achieve functional outcomes. Identify educational needs and provide PRN therapy as necessary. Serve as a clinical instructor or mentor for interns. Participate in hospital and discipline-specific skills assessments. Manage equipment orders and coordinate with vendors and outside professionals. Qualifications & Skills Bachelor's Degree in Physical Therapy required; Doctoral Degree preferred. Minimum of 1 year experience in rehab settings such as SNF, LTACH, or acute care preferred. Current BLS certification and valid GA PT license or eligibility.
