## Job description

Role Overview: A physical therapist responsible for assessing, planning, and implementing rehabilitative programs to improve patient mobility, reduce pain, and prevent deformities. Services are delivered across the continuum, emphasizing quality, cost-effectiveness, and regulatory compliance. Key Responsibilities Patient Assessment: Collaborate with physicians to develop and evaluate treatment plans, goals, and discharge strategies, incorporating patient and family input. Treatment Planning & Care: Implement and adjust treatment plans, select appropriate activities, provide patient education, and coordinate follow-up and community referrals. Documentation: Accurately document evaluations, treatment sessions, education, and discharge information per departmental standards. Professional Conduct: Uphold ethical practices, maintain confidentiality, punctuality, dress code, and effective communication. Participate in meetings, process improvements, and workload organization. Customer Service & Teamwork: Foster a positive environment, apply service recovery principles, and demonstrate respect and collaboration with patients and staff. Professional Development: Engage in ongoing education, complete required competencies, participate in training, and maintain licensure. Qualifications & Skills Master's in Physical Therapy (Doctorate preferred) Valid PT license and BLS certification Minimum one year of experience in rehab, LTACH, acute care, or SNF Effective communication, customer-focused attitude, teamwork, and basic computer skills Additional Requirements: Ability to perform other duties, adhere to policies, and demonstrate professionalism in all aspects of work.
