Treatment Planning & Care: Implement and adjust treatment activities, provide patient and family education, discharge instructions, and referrals to community resources. Serve as a resource for staff and physicians regarding rehabilitation services. Documentation: Record evaluations, treatment sessions, family education, discharge plans, and ensure timely submission according to departmental protocols. Professional Conduct & Customer Service: Uphold ethical standards, maintain confidentiality, punctuality, and dress code. Communicate effectively, foster teamwork, and ensure a positive environment for patients and staff. Professional Development: Engage in ongoing education, complete competencies, participate in orientation, and adhere to mandatory requirements to maintain licensure and skills. Qualifications & Skills Masters in Physical Therapy required; Doctorate preferred. Licenses: Active Physical Therapist license and BLS certification.
