## Job description

Role: The Physical Therapist assesses, plans, and implements rehabilitative programs to improve mobility, reduce pain, increase strength, and prevent deformity in patients with diseases or injuries. Services are provided across the continuum with a focus on quality, cost-effectiveness, and regulatory compliance. Responsibilities: Conducts patient assessments, collaborates with physicians to develop and evaluate treatment plans, and establishes discharge goals. Implements and revises treatment activities, provides patient and family education, and offers follow-up and community resource referrals. Documentation: Records evaluations, treatment plans, and session details according to departmental protocols. Ensures timely submission of all required documentation. Professional Conduct: Upholds ethical standards, maintains patient confidentiality, communicates effectively, and participates in staff meetings and ongoing education. Adheres to infection prevention and safety protocols. Customer Service & Teamwork: Demonstrates a patient-centered approach, fosters a positive environment, and collaborates with team members to enhance patient experiences. Professional Development: Engages in mandatory training, maintains licensure, and participates in orientation and training activities. Qualifications: Must hold a graduate degree from an accredited physical therapy program (Masters or Doctoral). Required licenses include a Physical Therapist license and BLS certification. Effective communication, customer service orientation, teamwork, and basic computer skills are essential.
