## Job description

Role Overview: The Hospice Admission RN is a licensed professional responsible for educating patients and families about hospice services, performing assessments, and coordinating care to support terminal patients in their homes, assisted living facilities, or skilled nursing facilities. Key Responsibilities Educate families on hospice options, benefits, and financial responsibilities. Perform comprehensive head-to-toe assessments to determine hospice eligibility. Coordinate resources, order necessary equipment, and develop individualized plans of care. Obtain verbal physician certification, initial orders, and align with interdisciplinary team members. Provide emotional support, discuss advance directives, and facilitate transitions to hospice care. Develop care plans, determine visit frequencies, and ensure safe, effective symptom management. Educate families on contact procedures, after-hours care, and emergency protocols. Document findings, review medical records, and evaluate medication effectiveness. Qualifications & Skills Associates or Bachelor's Degree in Nursing; hospice experience preferred. Current RN license, BLS certification, valid state driver’s license, and private vehicle insurance. Minimum two years of acute care experience; hospice experience preferred. Strong empathy, compassionate communication, and consultative skills. Knowledge of nursing standards, ethical practices, and patient-centered care models. Additional Requirements: Must adhere to organizational policies, demonstrate professionalism, and support service excellence in hospice care delivery.
